What Louisiana requires
- No person or firm may service portable fire extinguishers without a current license from the State Fire Marshal (each operating location of a firm holds its own firm license), and each technician carries the license while working and shows it on demandLa. R.S. 40:1664.4 (A) · La. R.S. 40:1664.7 (A)
- A firm that inspects, installs, maintains or services portable fire extinguishers holds a certificate of registration with the endorsement for that work before doing it, and every tag shows the firm's State Fire Marshal certificate numberLAC 55:V.3009 (A); 3039 (Service Tags) 6 (c)
- After a portable extinguisher is certified, hydrostatically tested, inspected, installed, maintained or serviced, a completed service tag showing all work done goes on at the valve or neck or the gauge (never the hose). It reads "DO NOT REMOVE BY ORDER OF THE STATE FIRE MARSHAL" and gives the firm's name, address, phone and certificate number, the technician's name, license number and signature, the day, month and year punched, the type of work punched (Installation, Certification or Service) with the specifics on the back, the serial number, and the owner and owner's address on the backLAC 55:V.3039 (Service Tags) 1, 3, 6 · LAC 55:V.3031 (A) (2)
- Every service, yellow and red tag gives the servicing firm's name, address and telephone numberLAC 55:V.3039 (Service Tags) 6 (b)
- A portable extinguisher that is inoperable for any reason is red tagged, with each impairment written on the back, and the firm gives written notice (email or fax allowed, on the Fire Marshal's form) to the owner and to the Office of State Fire Marshal Inspection Section as soon as practical and within two working days; the local fire department should also be toldLAC 55:V.3039 (C) (3), (6), (7)
- The written notice to the State Fire Marshal gives the owner's name, address and phone; the firm's name, address, phone and certificate number; the technician's name and license number; the type of equipment (manufacturer and model); the code, inspection chapter and edition used; the reason for the impairment with a copy of the inspection or service report; and the date it was red or yellow taggedLAC 55:V.3039 (D)
- A yellow partial impairment tag marks a minor deficiency on equipment that still works (including routine service the owner has not approved); it may stay on no more than 60 days, after which the firm notifies the State Fire Marshal Inspection Section in writingLAC 55:V.3039 (Partial Impairment Tags) 1 to 4
- Each time an extinguisher is opened, NFPA 10 maintenance is done and a new external verification collar (any color but yellow or red) goes on at or below the valve with the firm's certificate number, the technician's name and license number and the punched month and year; a six-year maintenance is recorded on a durable metallic or equivalent decal on the shell with the year and month, the firm's pre-printed name and certificate number, and the technician's initials and license numberLAC 55:V.3031 (A) (5), (B) to (G)
- When an extinguisher is taken off the owner's premises for service, the firm leaves a replacement of equal or greater rating, one for one, unless the building has more extinguishers than NFPA 10 and NFPA 101 require; a unit that cannot pass hydrostatic testing is red tagged or removed from service and destroyedLAC 55:V.3031 (A) (3), (4)
- Portable extinguisher service tags may be any color but yellow or red, are 5 1/4 by 2 5/8 inches on card stock, plastic, vinyl, Tyvek or metal, are printed and a sample sent to the State Fire Marshal's Licensing Section for approval, may carry a preprinted signature (portable extinguisher tags only; apprentices may not sign tags), and may be removed only by licensed employees of a certified firm, the State Fire Marshal's Office or certified fire prevention bureausLAC 55:V.3039 (Service Tags) 2, 6 (e); (F) (2) to (5)
Licenses and certificates
The firm needs a State Fire Marshal license and a certificate of registration with the portable extinguisher endorsement, and every technician needs a license, carried on the job. The firm's certificate number and the technician's license number go on every tag.
The service tag
After any inspection, maintenance or service, a completed tag goes on at the valve or neck or the gauge, never the hose. It reads DO NOT REMOVE BY ORDER OF THE STATE FIRE MARSHAL and shows the firm's name, address, phone and certificate number, the technician's name, license number and signature, the punched date, and the type of work punched as Installation, Certification or Service, with the details, the serial number and the owner's name and address on the back.
Portable tags may be any color but yellow or red. Every time an extinguisher is opened, a new verification collar goes on below the valve, and a six-year maintenance gets a durable decal on the shell.
Yellow tags, red tags and the Fire Marshal's notice
A unit that is inoperable for any reason gets a red tag listing each impairment, and the firm sends written notice to the owner and the State Fire Marshal's Inspection Section within two working days. A unit with a minor deficiency that still works can get a yellow tag for up to 60 days; after that the Fire Marshal is told in writing.
The notice gives the owner, the firm and its certificate number, the technician and license, the equipment, the code edition used, the reason and the date tagged, and it includes a copy of the inspection or service report.
From the walkthrough to a finished Louisiana report
Note each unit's serial number, whether the visit was an installation, a certification or a service call, what you did and which tag went on. ReportDraft lists the tags by serial number with your certificate and license numbers in the header, so the report is ready to send with a red or yellow tag notice.
Common mistakes
- Punching "Service" for an annual inspection (that is "Certification").
- Hanging the tag on the hose instead of the valve, neck or gauge.
- Using a yellow or red service tag on a portable extinguisher.
- Missing the two working day notice after a red tag.
- Leaving a yellow tag on past 60 days without telling the Fire Marshal.
