Every injury to the insurer, within 5 days

Report all work injuries to your insurance carrier or third-party administrator within five days of the injury, or of the day the employee reported it, whichever is later. The insurer then files the Division report for you.

The Division report, within 30 days

Any injury or death needing more than immediate first aid, or causing further treatment or lost time, gets a full report to the Division within thirty days: the employee's name, address, date of birth and wages, the time and cause of the accident, the nature and extent of the injury, medical costs and lost work time. Knowingly failing to report, or reporting falsely, is a misdemeanor. The reports are confidential.

The employee's written notice

Employees give written notice within thirty days of the accident, or of diagnosis for an occupational disease or repetitive trauma, stating the time, place and nature of the injury and their name and address.

From field notes to a finished Missouri report

Note the time and cause, the nature and extent of the injury, the treatment and any lost time, and when the employee told you in writing. ReportDraft writes the incident up with those details and keeps fields for the written notice, the lost time and the insurer report.

Common mistakes

  • Not telling the insurer about an injury because it looked minor.
  • Missing the five-day insurer window.
  • Leaving lost work time and medical costs out of the report.
  • Leaving the cause or the extent of the injury vague.
  • No written notice on file from the employee.
