  • The record gives the date and the time, to within at least half an hour, when the application started or stopped, kept so it is clear which; an entry of only A.M. or P.M. is not enough8 CCR 1203-2, Part 6.03 (i)
  • The record gives the name and license number of the person who made or supervised the application; a technician's restricted use application also names the responsible on-site qualified supervisor8 CCR 1203-2, Part 6.03 (j)

  • Post-construction termite liquid and bait treatment records add a diagram, blueprint or plat with approximate measurements, current and previous activity, conducive conditions and the areas to be treated and how, the signed contract and warranty, a label copy, a qualified supervisor's signed approval of the treatment diagram, and for baiting the number and locations of stations and every service inspection; pre-construction treatments record the square footage, equipment flow rate and start and stop times8 CCR 1203-2, Part 6.04

  • Restricted use pesticides are used only by a licensed qualified supervisor, certified operator or private applicator, or by a technician under the on-site supervision of a qualified supervisor, and only in a category the licensee holds8 CCR 1203-2, Parts 15.02 to 15.04
