The pesticide use record
After every structural pesticide application, and after a termite system is installed or changed, the owner, resident or custodian gets a legible record. It gives the customer's name and the address treated, the company's name and phone, the applicator's name with their registration or certification number, the specific areas treated, each product by brand name with its EPA registration number, amount, concentration and method, the target pests, the date, and any precautions to follow afterwards.
For institutions, schools and commercial facilities, it also records the start and completion times of the application. The company keeps its records for at least two years and shows them to the Department on request.
Sending records electronically
The record can be on paper or electronic. To send it electronically, the property owner or custodian first signs the electronic communication acknowledgment form in the wording the rule sets out, and the company keeps that form for as long as the contract runs or two years after it ends.
Contracts, vehicles and safe use
Every structural pest control service, including general pest and rodent control, is done under a contract. Service vehicles carry the company's full name and a spill control kit, the company shows its business license on request, and certified operators instruct the technicians they supervise on choosing, mixing and using pesticides safely.
From field notes to a Georgia service record
Note each product with its EPA number, amount and concentration, where it went and what you told the customer. ReportDraft prints the company name and phone and the applicator's registration number, fills the products table, records start and completion times for commercial accounts, and lists every post-application precaution.
Common mistakes
- Leaving the EPA registration number, amount or concentration off a product.
- Missing the applicator's registration or certification number.
- Not recording start and completion times at a school or commercial account.
- Emailing records without the customer's signed electronic acknowledgment form.
- Forgetting the post-application precautions.
