Illinois regulates structural pest control through the Department of Public Health under the Structural Pest Control Act, and the Structural Pest Control Code, 77 Ill. Adm. Code 830, lists what each application record holds, down to the percentage of active ingredient in the finished product. Here is each rule with a link to the official text, and what an Illinois pest control service record should carry.

  • Records give the name of the technician applying the pesticide and the date and time of the application77 Ill. Adm. Code 830.820 (d) (2), (3)

  • Termite control records include a diagram showing foundation types and accurate dimensions of all portions of the structures treated77 Ill. Adm. Code 830.820 (d) (6)
  • The certified technician responsible for non-certified personnel documents their review of every pesticide record for services those technicians performed, with name, certification number and dates of service77 Ill. Adm. Code 830.820 (c)
  • Commercial locations keep records of all pesticide applications at least two years, restricted pesticide records separate from general ones, at the business location (except the past week's) for Department inspection, and give copies to the property's current owner, manager or tenant on request77 Ill. Adm. Code 830.820 (a), (d), (e), (f)
  • Every non-certified technician works under a certified structural pest control technician at the location, who must be physically present at the work site when the label requires it or until the technician has had at least two supervised uses of each type of pesticide at each type of work site77 Ill. Adm. Code 830.270
  • Service vehicles carrying pesticides are locked when unattended, with restraints for containers, absorbent material for spills, protective equipment, a first aid kit with poison control numbers, a label for each pesticide carried, and a complete change of clothing including shoes77 Ill. Adm. Code 830.840
  • Schools and early care and education centers give registered parents, guardians and employees (or all parents and guardians) written or telephone notice at least 2 business days before a pesticide application, except in an imminent threat; disinfectants and insect and rodent baits are exempt225 ILCS 235/10.3 (as amended by P.A. 104-480, effective 2026-07-01)
  • Each commercial structural pest control business location needs its own Department license, displayed prominently with the certificates of the technicians working there225 ILCS 235/4 (a) ยท 77 Ill. Adm. Code 830.310

The record, item by item

Record the customer's name and address or the site, the technician who applied the pesticide, the date and the time, and the target pest or purpose. For every product, give the brand or common name, the USEPA registration number, the percentage of active ingredient in the finished product, and the amount of finished product used, estimated to within 10 percent.

Termite jobs add a diagram of the foundation types with accurate dimensions of every part of the structure treated.

Uncertified technicians and record review

A non-certified technician works under a certified technician at the location, who must be on site when the label requires it or until the technician has done at least two supervised uses of each pesticide type at each type of site. The certified technician documents a review of the uncertified technician's records with their name, certification number and dates of service.

Keeping and sharing records

Keep application records at least two years, with restricted pesticide records separate from general ones, at the business location for the Department to inspect. A property's current owner, manager or tenant who asks gets copies of the service records.

Vehicles, licenses and schools

Service vehicles stay locked when unattended and carry restraints, absorbent, protective equipment, a first aid kit with poison control numbers, a label for every product on board and a full change of clothes including shoes. Each business location holds its own license, displayed with its technicians' certificates. Schools and early care and education centers give parents at least 2 business days' notice of applications, though baits and disinfectants are exempt.

From the truck to a finished Illinois record

Note the time, each product's finished percentage and roughly how much you used, and what you were after. ReportDraft fills each line Illinois requires, adds a reviewer line when a trainee did the work, and reminds you that termite jobs need the foundation diagram.

Common mistakes

  • No time of application on the record.
  • Giving the concentrate's percentage instead of the finished product's.
  • Mixing restricted pesticide records in with general ones.
  • A termite treatment record with no foundation diagram.
  • No documented review of a trainee's records.
