- A pest control operator or responsible managing employee keeps records on all jobs performed for at least two years, each with the date of the job, the client's name and address, the branch of pest control, and the chemicals and treatment methods usedHAR 16-94-25.5 (am. and comp. 2022-02-17)
- A licensed RME directly manages the pest control business (supervising projects, technical and administrative decisions, personnel, contracts and compliance), and pest control field representatives are licensed to secure work, identify infestations, inspect, bid and sign contracts for a licensed operator; licenses are not transferableHAR 16-94-3, 16-94-26
- Certified applicators keep records of all restricted use pesticide applications at their principal place of business for two years: the brand or common name, EPA registration number, type of formulation, percent active ingredient, target pest, dilution rate, total amount used, total area covered, time and date, address or location, the certified applicator's name and certification number, the site, and the restricted entry interval and whether posting and oral notification are requiredHAR 4-66-62 (a)-(c) (am. and comp. 2019-08-23)
- Every certified restricted use pesticide applicator files an annual report of all restricted use pesticide use within 30 days after the calendar year ends, with each product's EPA number, name, active ingredients, total quantity and total area, the dates, and the location including at least the tax map key numberHAR 4-66-62 (f)
- No restricted use pesticide may be applied on or within 100 feet of a school's property (preschool through high school) during normal school hours, Monday to Friday 7:00 a.m. to 4:00 p.m. on days classes are in session; whole structure fumigation is exceptedHAR 4-66-64.2
- Certified applicators create or verify records showing that each non-certified person who mixes, loads, transfers or applies restricted use pesticides has the required qualifications; failing the recordkeeping rules can cost the certificationHAR 4-66-62 (h)-(i)
- A termite inspection report (TIR) goes on the Board's PC-9 form with a foundation diagram showing where termite evidence and damage were found, a statement of the inaccessible areas not inspected and why any accessible area was skipped, and recommendations for treatment; it is done by a branch 1 or branch 3 operator or RME who signs it (a field representative may inspect under their supervision), checking the exterior, crawl space, interior and attic for evidence and conducive conditionsHAR 16-94-70, 16-94-71, 16-94-73
- A termite control service agreement is in writing, signed by the licensee and the client, with the inspection frequency, the fee per inspection, the expiration date, the type and manner of treatment, whether the fee includes repairs, and whether treatment can be done at the inspection and at what extra charge; the original inspection report and later control service reports are kept at least two years after the agreement endsHAR 16-94-72
- Company service vehicles show the company's name and pest control operator's license number, and so does every advertisement; 'termite proof' and 'government approved' may not be used, 'guarantee' only with a statement of its limits and the work covered, 'free inspection' only with a written report, and 'bonded' only with the bond's terms and payeeHAR 16-94-80 to 16-94-86
