  • Each customer gets a statement of services or contract with the business's name, address and license number, the customer's name and address, the pests, each pesticide with the quantity applied and total area, the concentration or rate, the date, location and start and end times, the expiration date of any guarantee, the signature and applicator certification number of the individual who performed the service and of any supervisor, the wind direction and velocity when applicable, and a note when the application was less than the label rateK.S.A. 2-2455 (a)
