  • Each application record gives the trade name as it appears on the label, the target area, the target pest (for a mixture, which pest each pesticide targeted), the total amount applied, the percentage use dilution and the application rateTenn. Comp. R. & Regs. 0080-09-04-.06 (3) (b), (e) to (i)
  • For each restricted use pesticide application, the record also gives the EPA registration number and the time of applicationTenn. Comp. R. & Regs. 0080-09-04-.06 (3) (k)
  • Applicators give the property owner or tenant a copy of the application record within 48 hours of a request, and keep records for two years from the date of applicationTenn. Comp. R. & Regs. 0080-09-04-.06 (4), (5)
  • Before treating for wood destroying organisms, the company executes a written contract in duplicate with the inspection date and a graph of the property showing visible insects or damage, a transfer provision for later owners or renters, and a unique contract number; termite contracts include a one-year retreatment warranty, a contract without a damage guarantee says so in half-inch letters on the front, and contracted properties are inspected every yearTenn. Comp. R. & Regs. 0080-09-04-.08 (3) (a)
  • Termiticides are mixed and applied within a 10% variance of label directions; a less than complete treatment requires telling the customer the possible outcomes, a bold note in the contract initialed by the owner or tenant, and marking the untreated areas on the graph; new construction pretreatments are finished within 90 days after backfill unless the label says otherwiseTenn. Comp. R. & Regs. 0080-09-04-.08 (3) (b) 1.
  • Before treating active wood destroying beetles, the applicator explains the risks and alternatives such as replacing infested wood; a preventive beetle treatment of an existing structure is allowed only when at least five tested areas show wood moisture of at least 18%, with a written report of the findings for the contractTenn. Comp. R. & Regs. 0080-09-04-.08 (3) (b) 2.
  • For a fumigation with a pesticide poisonous to humans, a FUM or FUS licensee is present and in charge, a gas mask is on hand for each person, warning signs are posted at every entrance, guards keep people out, and the site is cleared of fumigant per the label before re-entryTenn. Comp. R. & Regs. 0080-09-04-.08 (1)
  • Applications that need certification are made only by a certified applicator or under the direct supervision of one certified in that category, always consistent with the label; applicators keep records available for inspection and give full information on how and with what any application was madeTenn. Comp. R. & Regs. 0080-09-04-.09 (1)
