• For every pesticide applied, general or restricted use, the record gives the date, the sites treated and principal pests, the area treated, the brand or product name on the label, the EPA registration number, and the amounts of concentrate and diluent used, by weight or volume2VAC5-680-65 (3) to (8)
• Pesticide businesses keep all records for two years, open them to the commissioner during normal business hours on written request, and send records not readily available within 72 hours of a written request2VAC5-680-50
• Certified applicators and registered technicians report any pesticide accident or incident that threatens people, public health or the environment to the Office of Pesticide Services by telephone within 48 hours, then in writing within 10 working days with the people involved, the pesticide, quantity spilled and containment, time, date and place, mitigation, and any nearby water at risk2VAC5-685-170
• Anyone mixing or applying pesticides commercially in Virginia holds a pesticide business license or works for a licensed business; the business license does not replace the commercial applicator's certification2VAC5-680-30
